چکیده

We report a case of sarcoidosis presenting with cor pulmonale of a greater severity than would be expected from the degree of hypoxia and pulmonary fibrosis. Right heart catheterization revealed that mean pulmonary artery pressure was markedly increased (42 mm Hg), was not reduced by supplemental oxygen, but was reduced significantly (25 mm Hg) after 16 weeks of therapy with corticosteroids. Improvement in symptoms and pulmonary function was maintained for an 18-month period of observation after cessation of corticosteroid therapy.

Pulmonary sarcoidosis.

Sarcoidosis is a granulomatous disease of unknown cause, occurs worldwide and has a highly variable prevalence. The disease is typically dominant in the lungs, although it can affect virtually any organ and is unpredictable in its clinical course. The severity of pulmonary sarcoidosis ranges from incidentally discovered radiographic abnormalities in asymptomatic patients to a chronic progressiv...

Pulmonary hypertension associated with sarcoidosis

Pulmonary hypertension (PH) affects approximately 6% of unselected patients with sarcoidosis but its prevalence is much higher in advanced pulmonary fibrosis. Although destruction of the distal capillary bed and resultant hypoxaemia are central, the underlying mechanisms are multifactorial: left heart dysfunction, specific pulmonary vasculopathy (which can cause pulmonary veno-occlusive disease...

Pulmonary hypertension in advanced sarcoidosis: epidemiology and clinical characteristics.

Pulmonary hypertension (PH) is a predictor of poor outcome in sarcoidosis. Little is known about the epidemiology of PH in sarcoidosis. The current authors reviewed the records of patients with sarcoidosis listed for lung transplantation in the USA between January 1995 and December 2002. PH was defined as a mean pulmonary artery pressure of >25 mmHg and severe PH as a mean pulmonary artery pres...

Hepatic Sarcoidosis

Sarcoidosis is a multisystem disease characterized by the presence of non-caseating granulomas in affected organs. Pulmonary involvement is the most common site of disease activity. However, hepatic involvement is also common in sarcoidosis, occurring in up to 70% of patients. Most patients with liver involvement are asymptomatic. Therefore, the majority of cases are discovered incidentally, fr...
